PPS Annual Review Form Parent form - templateA statement of special educational need must be reviewed by the Local Authority every 12 months. In order form them to do this, the school must hold an annual review meeting to look at the statement and make any recommendations for changes. As part of this process, parents have the opportunity to give their views on progress and/or concerns. This template letter may help you structure your report. It can be completed on a computer, be saved and be printed out and signed before giving it to the school.
